Identification:
All identification must be the original issued and current. The following forms of identification are acceptable:
• State Driver's License
• U.S. Military ID card
• State Identification Card
• U.S. Passport
• Foreign Passports Stamped by U.S. Customs

The name you will be signing on the document must match the name on your ID.

What if I do not have identification, can I still get a document notarized?
Yes, by having Credible Witness.
Credible Witnesses
If you do not have proper Identification available, or the name on your ID is different due to a recent marriage then you may use two credible witnesses to have a document notarized.
Two Credible Witnesses :

must know the signer by the name on the document to be notarized
-
can and is willingly to swear to the signer's identity
-
not named on the document to be notarized
-
does not have a financial interest in the document to be notarized

has Proper ID (listed Above)
-
must be 18 or older
Signing a signature with a mark

If individual is not able to write their signature and has Valid ID:
1. two “disinterested” witnesses need to be present and sign as witnesses on the document
2. the person that is getting the document notarized will make their signature mark
​
3. one witness will write the person’s name next to their mark
​
then will sign his or her name as a witness.

4. the second witness will verify that they witnessed the individual make his or her mark on the document
